The Aadhaar enrolment form is available at the Enrolment Centres or can be downloaded online from the UIDAI website. It’s a two-page document with the first page that needs to be filled and the second page has instructions for reference while filling up the form.

The Aadhaar Enrolment Form can also be used in case an individual wants to change or correct any of the details. In this scenario the 12-digit Aadhaar number needs to be entered and fill up those columns that needs to be corrected.

How to download Aadhaar Enrolment Form Online?

Step 1: Visit the UIDAI website  as shown in the screenshot below.

Step 2: Click on Resources option and select “Download Forms’

Step 3: Click on the ‘PDF’ icon to download the Enrolment Form to save it on the computer and take print of it.

Q. How to change name in aadhaar card?

A. You can apply for the name change, correction or updating Aadhaar Card online.

For online updates, you can follow the below mentioned steps

  • Go to UIDAI website
  • Enter your 12-digit Aadhaar number, enter the OTP and then proceed
  • Select the option of changing or updating the Aadhaar details, update them and move to the next step
  • Upload supporting documents such as PAN card, driving license and so on
  • You will receive a confirmation number and a URN. Keep this number safe for tracking the status of the name change in Aadhaar card.

Note: You will have to keep your registered mobile number with you while updating the name online as you will receive OTP on this number.

For updating name in Aadhaar card via Post or enrolment center

  • Download the PDF form from this link
  • Fill in the required details
  • After completing the form, you can either submit it at the nearest enrolment center or put it in an envelope and post it

    (a) UIDAI, Post Box No. 10, Chhindwara, Madhya Pradesh – 480001, India

A. If you want to make corrections in your address present in your Aadhaar Card or want to update with a new address you can do that by yourself through the official online Aadhaar card portal.

The first thing that you have to do is click on the link and then follow the below mentioned steps to complete the process.

  • Enter your 12-digit Aadhaar number
  • Fill in the text verification number
  • Then click on Send OTP (One Time Password)
  • You will receive a six-digit OTP on the mobile number registered with your aadhaar card
  • Enter the OTP and proceed
  • Select the change address field and fill in the new address details
  • In the next step, you will be required to submit the necessary address documents.  After submitting it, you will be given a URN number
  • You can either note down the URN number or simply save it to track the status of your update request.

Q. How to change date of birth in aadhaar card?

A. You can change your date of birth in your Aadhaar card online and offline at post and enrolment centre.

Online Method

Note: Before opting for this method, you need to ensure your mobile number is registered with your aadhaar card.

  • Visit the UIDAI portal
  • Enter your aadhaar number and the verification code and select send OTP
  • Enter the OTP sent and select Date of birth option, followed by language
  • Upload the required documents and submit.
  • Note down the URN number, so that you can track the status of your update request.

Offline Method – By Post

  • Download the form(link to pdf) and fill out all the required details
  • Attach self-attested copies of all the relevant documents, mentioning your aadhaar number and registered mobile number.
  • Send the form and documents to your regional office. Your new card will be dispatched in 15-20 days.

Offline Method – Enrolment Centre

Visit the nearest enrolment centre and submit the completed form with all the relevant documents.  You will be given an URN, which you can use to track the status of your update request.

Q. How to change photo in aadhaar card?

A. The photo displayed on your aadhaar card can be changed in one of two ways:

  • Send your request to the regional UIDAI office– Download the aadhaar correction form and fill in the details. Attach a formal letter requesting a change to the photo displayed in your card.  Attach the new self-attested photo with the form and a copy of your aadhaar card and post it to your regional office, which will respond in 15-20 days with your new aadhaar card.
  • Visit your nearest UIDAI office – Visit the nearest aadhaar office and request the officer to re-do your biometrics, which carries a nominal fee.  Your new card will be mailed to you in 15-20 days.
